Nipple sparing mastectomy: Can we predict the factors predisposing to necrosis? Eur J Surg Oncol 2011;38(2):125-129. doi:10.10116/j.ejso.2011.10.007

The work included a prospective examination of 50 patients who underwent mastectomy and prophylactic mastectomy. The parameter of the degree of necrosis of the NAC complex after surgery was monitored. The result showed that there was no complete necrosis of the NAC complex. Only 18% of those included in the study needed additional surgical debridement of the NAC complex. Factors that influenced survival, which were included in the study, were smoking, patient's age, type of incision, and flap thickness. It was concluded that the mentioned factors affect survival with the recommendation that the flap must be thicker than 5 mm to preserve the NAC complex after NSS mastectomy.

  • The paper presents the possibilities of reconstruction of the lower third of the lower leg and foot. In the period from 2004 to 2011, at the plastic surgery department of the Clinical Center in Foča, 40 patients were operated on, in which 4 types of neurohutal flaps were used, proximally based superficial sural incision, proximally based lateral sural incision, distally based saphenous flap and distally based sural incision. superficial lobe. Defects occurred as a result of trauma, diabetes, radical tumor excision... Flaps were accepted completely in 35 patients, and complications occurred in 5 patients. The size of the lobes was from 4x3cm to 12x8cm. Conclusion: The advantages of using these flaps are easy discectomy, adequate thickness of the skin tissue and sparing of large blood vessels, while the disadvantages are the sacrifice of the sural or saphenous nerve, and the donor scar in young patients after the application of a free graft.

Sentinel lymph node in breast cancer patients: A comparative study from a single institution trial in Bosnia and Herzegovina. 15th Milan Breast Cancer Conference, Milan. Abstract book, 2013, D09
  • Sentinel biopsy of the lymph node in the axilla in early invasive breast cancer represents the standard in the operative treatment of breast cancer. The paper presents the first results of the study that was carried out in the Clinical Center of Banja Luka. 68 women were analyzed in the period from 2010 to 2012. Marking of the sentinel lymph node was done with radiopharmaceuticals and tissue dye methylene blue. In the examined group, marking was done using a dual method, and in the control group, only with tissue dye. The results showed that there was no significant statistical difference between the compared groups and that the sentinel biopsy procedure showed reliability in determining the status of other axillary lymph nodes.

  • Breast reconstruction after breast removal is very important for breast cancer patients. Secondary reconstruction is a procedure performed after mastectomy using endoprosthesis, autologous tissue, or a combination of these two methods. In this paper, the results of the research in the period from 2008 to 2014, during which 36 operations of TRAM and LD flaps of secondary reconstruction were performed, were processed. The application of these operations has shown that a well-planned operation, individually directed, and with the good experience of the surgeon, leads to a very good aesthetic result of the reconstruction.

  • Primary breast reconstruction is a breast reconstruction procedure at the same time as breast removal. Primary reconstruction can be done in two ways, using implants or expanders, and reconstruction in two acts. Postoperative complications were monitored from 2010 to 2015. 46 reconstructions were done. The early postoperative result is more than satisfactory. Early postoperative complications did not occur to a significant extent to affect the postoperative result.

  • The results indicate secondary breast reconstruction procedures, which are performed with different techniques (latissimus flap, TRAM flap, silicone implant and tissue expander) in order to reconstruct the breast after mastectomy due to malignant disease. The results did not indicate the existence of a significant statistical difference between the different applied procedures for the purpose of secondary (delayed) breast reconstruction.

  • The work includes an examination of the complications of primary breast reconstruction that were performed in the UKC RS in the period from 2011 to 2016. Primary breast reconstruction is done in three ways using own tissue, the so-called. by applying flaps; using an implant or using an expander in combination with an implant. Complications of primary breast reconstruction are partial or total flap necrosis, seroma, hematoma, asymmetry, infection, capsular contracture and hypertrophic scar. The conclusion is that, with an insight into the complications, this operative technique is an acceptable option for primary breast reconstruction in targeted patients.

  • The reconstruction of the NAC complex /nipples, areola complex/ belongs to the final stage of breast reconstruction. There are several ways to reconstruct the NAC complex. The paper shows the experience of applying the "butterfly" flap in combination with areola tattooing. With careful work and a well-planned operation using the mentioned flap, a very good aesthetic and medically acceptable result is achieved.

  • Cilj rada je bio da se prikaže značaj, učestalost i složenost liječenja kožnih tumora dijagnostikovanih i liječenih u Klinici za plastičnu hirurgiju u Banjaluci u desetogodišnjem preiodu. Od ukupnog broja 2311 operisanih pacijenata, 1210 pacijenata operisano zbog kožnih neoplazmi različite etiologije. Najčeći su bili tumori u regiji glave, 15,7% operisanih. Iz navedenog rada se uočava porast broja operisanih pacijenata, te učestalost kožnih tumora posebno na fotoeksponiranim regijama kao što je slučaj u sličnim relevantnim svjetskim studijama.

  • The aim of the work was to present the results of treatment of patients with rare forms of skin cancer and metastatic skin cancer operated on at the Clinic for Plastic Surgery in Banja Luka. The results of patients operated on for adenocarcinoma of the skin, most often localized in eyelids, hydroadenocarcinoma of the skin of eccrine and apocrine sweat glands, and Merkel cell carcinoma, Carcinoma trabeculare, are presented. Of the cutaneous metastases, breast, stomach, uterus, lung, and colon cancers were the most common, and prostate, testicle, and ovary cancers were rare.

  • The aim of the paper was to present the basic postulates of skin tumor prevention. The basic etiological factor is the cumulative effect of ultraviolet radiation, and photoprotection is the most important procedure that prevents the formation of permanent skin damage. Another important preventive procedure is the removal of all suspicious skin lesions (preventive excision) and their pathohistological examination, whereby early detection of the disease is equal to cure. Knowing the basic etiological factors and adequate prevention can greatly reduce the occurrence of skin tumors.

  • The paper shows the experience of treating tumor diseases of the eyelids and methods of reconstruction at the Plastic Surgery Clinic of KC BL in a ten-year period. In the mentioned period, 65 patients were operated on, of which 48% had a tumor localized in the medial corner of the eye, 3% in the lateral corner of the eye, 34% of those operated on had a tumor on the lower eyelid, and 9% on the upper eyelid. Pathohistologically, in 96% of those operated on, the most common tumor was Ca. Baseocellulare, and in 2% of cases it was Ca. Planocellulare and 2% Melanoma. Glabellar, McGregor and Mustarde flaps were most often used, while in the case of major damage, complex surgical interventions of cartilage transplantation and canthus reconstruction were performed. Eyelid reconstructions are one of the most complex ophthalmoplastic procedures, and a well-planned operation with respect for atraumatic work and technique achieves satisfactory aesthetic and functional results.

  • The aim of the work is to present our own results in solving the problem of soft tissue defects of the knee and lower leg, especially its distal third, considering its weaker circulation. The work covered the treatment of 30 patients in the period from 2004-2010 with the mentioned defects that were the result of trauma, complications of diabetes, decubitus ulcers, and defects after excision of malignant tumors. The flap was accepted in 26 patients, while minor or major complications occurred in others. The conclusion is that the advantages of these flaps are: quick and easy dissection, the thickness and quality of the flap, as well as the absence of sacrificing the main blood vessel of the lower leg, while the disadvantage is the sacrifice of sensitive nerves of the n.suralis and n. saphenous vein with donor scar in younger patients.

  • The aim of the work is to determine the number of patients operated on, as well as operative procedures for the reconstruction of lower leg and foot defects at the Clinic for Plastic Surgery of the Clinical Center in Banja Luka in the period from 2007 to 2011. The total number of patients was 124 patients. The largest number of patients were over 60 years of age (43 patients), and 72% were men. The most frequently used defect reconstruction technique was the application of skin autograft in 77 patients, and of the 19 pedicle flaps, 17 were sural flaps. The functional results after the performed surgical procedures are good, the aesthetic result is satisfactory, and the complications were minimal, mostly accompanied by partial necrosis of the flaps, which were treated secondarily with a skin autograft.

  • The aim of the work was to determine the number of operated patients, the level of traumatic amputation, the application of a local flap when covering defects in operated patients in the Emergency Surgical Outpatient Clinic in 2010. 144 patients were operated on, of which 120 patients were male, and 61 patients were older than 50 years. The most common amputated finger, regardless of the level of the amputated phalanx, is the third finger (in 47 cases), and the most frequently used flap is the v-Y Atasoy flap. In the conclusion, it was stated that an enviable number of traumatic amputations were performed for the number of treated patients, and that the operative procedures satisfied the aesthetic and functional result with minimal complications related to the complications of the flap itself.

  •  The aim of the work was to examine the effectiveness of partial fasciectomy, which was performed in 138 patients with an average age of 62 years and an implied follow-up period of 5.6 years. Postoperative complications included seroma, hematoma, partial and total necrosis of the flap occurred in 13.8% of patients, recurrence of contracture occurred in 7.2% of patients, and nerve lesion occurred in 4.3% of patients. The conclusion is that, taking into account the results, partial fasciectomy is an operative technique with a good final result, a low risk of disease recurrence and a slightly increased risk of nerve damage.

  • The paper analyzed the result of the application of LD incision in secondary breast reconstruction, as one of the methods of reconstruction applied at the Clinic for Plastic Surgery. The analysis was performed on 7 patients who were operated on at the Clinic for Plastic Surgery of KC Banja Luka. The results were compared with world standards in terms of the aesthetic effect as well as described complications. Despite the modest result of the operated, it was shown that the method is very reliable and the results are satisfactory.

  • The aim of the paper is to present and analyze the first results of lymph node biopsy in cutaneous melanoma. The analysis was performed in 44 patients over a period of three years who were administered the radionuclide Tc-99 and methylene blue. The results showed that in 45% of the operated patients, the catchment area was the axilla, and in 41% the catchment area was the inguinum. In 11% of those operated on, two catchment areas were registered. The definitive PH test was positive in 29.5%, negative in 70.5% of the operated and false positive in 4.5% of the operated. SLNB is absolutely acceptable and modern in diagnosing micrometastases of melanoma, which prevents lymphogenic metastasis.

  • The aim of the work is to show the possibilities of nose reconstruction divided into nine aesthetic subunits by applying local and regional flaps in order to achieve the best possible aesthetic result. In a period of 4 years, 56 patients were operated on and local nasal and regional flaps were used in reconstruction after skin tumor excision. Demographic data were recorded. The results showed that the most frequent tumor was ca. baseocellullare cutis and the most commonly used flaps are Rintala flap, bilobar flap, island nasolabial flap type I, II and III, glabellar flap, and frontal and supratrochlear island flap. The conclusion is that the choice of flap application is individual, and the goal is to get the best possible aesthetic result, which depends on the knowledge of the anatomy, the size and thickness of the defect, the patient's wishes and the surgeon's experience.

  • Based on the recommendations of the European Association of Mastologists (EUSOMA) and the decisions of the Parliamentary Assembly of the European Union, the position has been taken that breast pathology is dealt with by multidisciplinary units formed specifically for that purpose. The paper presents the results before and after the formation of the Breast Center at the University Clinical Center Banja Luka. The results showed progress, both in terms of operative technique and in terms of the introduction of new diagnostic procedures (wire marking, sentinel procedure, tissue needle biopsy of the breast, specimen mammography, etc.).

  • Breast reconstruction is an operative procedure that reconstructs a breast that was previously surgically removed. We distinguish between primary (immediate) and secondary (delayed) breast reconstruction. Complications arising from both operative procedures are presented in the paper. The obtained results indicated that there was no significant statistical difference in postoperative complications between primary and secondary breast reconstruction.

  • The aim of the work is to demonstrate the importance of perforator flaps in the reconstruction of soft tissue defects. In a period of three years, 39 patients with defects were operated on, in which perforator flaps were used. Demographic data are recorded in the work. as well as localization, defect size and complications. The conclusion is that the perforator flaps are easily and quickly raised, without raising the fascia and muscles and sacrificing the main arteries. They can be used for the reconstruction of minor and moderate soft tissue defects and are an alternative to free flaps.

  • Augmentation mammoplasty is an operative procedure that increases the volume of the breast and improves its shape. Postoperative complications after mammoplasty were analyzed in the paper, as well as the level of satisfaction of the patients with the achieved aesthetic result. Postoperatively, as a complication, seroma appeared in one patient, there were no infections, and on a scale of 1-5, the patients were on average satisfied with the aesthetic effect, and the result was 4.8.

  • The aim of the paper is to analyze the injection therapy of the use of hyaluronic acid fillers. The paper analyzed post-injection complications, and the period of satisfaction in 50 female patients with the aesthetic result, as well as the duration of the filler until complete resorption. Complications with increased swelling occurred in 20% of patients, but the swelling disappeared completely after ten days. Asymmetry was present in 10% of patients who also needed additional corrections. The duration of the filler was 7.3 months after the application. With the planned application of hyaluronic acid as a resorptive material for corrective aesthetic procedures of asymmetry or hypoplasia, it is possible to obtain a very high degree of satisfied patients without potential complications.

  • Corrective procedures are operative procedures, which most often correct breast asymmetry in a secondary operative act. Corrective procedures include mastopexy, reduction mastoplasty, augmentation mastoplasty and lipofilling. The aim of the work is the analysis of operational methods and the justification of their application. The paper analyzed the position of the new areolomammillary complex in relation to the fossa jugularis as an orientation point.

  • The aim of the paper is to show the frequency, experiences and operational possibilities in solving the problem of eyelid defects. The work includes follow-up over a fifteen-year period at the Clinic for Plastic Surgery, during which 118 patients were operated on. All demographic data were included in the work, and the data were analyzed, the frequency of tumors, of which 94% was Ca. Baseocellulre cutis. Complications were treated operatively according to surgical principles and the work showed that they did not affect the good aesthetic result and the functionality of the eye itself. A well-planned operation and the choice of a reconstructive procedure achieve more than satisfactory aesthetic and functional results.

  • The paper presents the treatment case of the female patient and the successful diagnosis of Gorlin Goltz Syndrome, the so-called. A syndrome that accompanies the multiple occurrence of basal cell carcinomas of a specific distribution and localization with associated anomalies that accompany the syndrome itself. The patient was operated on several times at the Plastic Surgery Clinic of KC RS, during which surgeries were performed to remove PH-verified cancers on various localizations on the body. The conclusion of the paper is that even if Gorlin Goltz syndrome is a rare disease, it requires very good diagnosis and serious surgical treatment and knowledge of the criteria itself enables good and easier diagnosis.

  • The work shows methods of operative treatment of patients who have suffered injuries to the tips of the fingers of the hand as a result of traumatic injuries. The paper shows the results of the application of the reconstructive procedures of the Atasoy flap, the Cross finger flap, and the thenar flap in treating defects of the tips of the fingers of the hand. Demographic data are included in the paper, and the result of treatment with the complications present is analyzed. The conclusion is that the application of flaps is very important in order to preserve the length of the fingers of the hand, and enable a good functional and aesthetic result.

 - Breast cancer predominantly occurs in women over 50 years of age. However, a number of breast cancers also occur in women younger than 40 years old. They are followed by a more aggressive and unpredictable form of the disease. The literature states that about 7% of breast cancer patients are younger than 40 years old. Based on the data from the Operative Protocol of the Breast Center UKC RS, in the period from March 2009 to December 2013, out of a total of 647 operated patients, 43 of them (6.65%) were under 40 years of age. In this group of patients, a more frequent occurrence of triple negative and HER2 positive breast cancer, which is normally characterized by a more aggressive biological form of the tumor, was recorded.
